How can I find out if I qualify for my FREE Lactation Visits?

Anthem, Blue Cross/Blue Shield, United and Multi-Plan:

We contract with The Lactation Network (TLN) for insurance covered visits with Anthem, Blue Cross, Blue Shield, United and other PPO plans with a multi-plan logos. Complete the insurance verification form using the link ABOVE and you will hear back from TLN regarding your lactation coverage - usually in less than 24 hours! (If you do NOT hear from TLN within 24 hours of submitting your form, PLEASE email us at hello@breastfeedoc.com so we can follow-up for you!)

Aetna Families -

We are an in-network provider for most Aetna PPO plans. Please email us through the link ABOVE (or directly to hello@breastfeedOC.com) to verify your insurance benefits! Send your name, date of birth, primarily policy holder name and date of birth, AND a copy of your insurance card (front and back).

Cigna Families-

We contract with Wildflower to offer Lactation Consultations to families with Cigna PPO, Cigna HMO, and Strategic Alliance insurance. To verify coverage simply fill out a short form using the link ABOVE! If you are not approved, please reach out to us for DISCOUNTED cash pay options.

Not covered through one of the above? You may still have lactation benefits!

Visits can still be booked and payment will be due upon booking. After your visit, you will be provided with a superbill which can be used to submit a reimbursement claim through your insurance provider. We recommend calling your insurance provider first to confirm coverage if out of network.

Home Visits may incur additional travel fees even if your consultation is covered through insurance. Reach out to us at hello@breastfeedoc.com for specific travel rates to your area.
